If you are a PMHNP (Psychiatric-Mental Health Nurse Practitioner) building a remote practice, the licensure puzzle is genuinely harder than it is for psychologists. You are working across two compacts at the same time: the Nurse Licensure Compact (NLC) for your RN license, and the newer APRN Compact for your nurse-practitioner privilege.

A PMHNP may need both the RN licensure foundation and APRN authority before practicing in another state. Holding only the NLC is not enough to assume prescribing or independent PMHNP practice.

As of mid-2026, the NLC is effective in approximately 42 jurisdictions. The biggest population states not in the NLC are still California, New York, Illinois, Hawaii, Alaska, Oregon, Nevada, and Connecticut — though several of these have legislation in motion.

APRN Compact states in 2026
APRN Compact status should be checked against official sources before relying on it. Earlier materials identified these enacted or early-adopter states:

- North Dakota
- Utah
- South Dakota
- Kansas

- (additional states pending implementation)
This is a much shorter list than the NLC because the APRN Compact is newer and more politically contested. Your home state being NLC-compact does not mean it's APRN-compact. Verify each state separately with the board of nursing and official compact materials.
The honest path for a US-wide remote PMHNP
If your goal is to be hired by a multi-state telepsychiatry company like Talkiatry, Brightside, Cerebral, or Rula, here is the realistic path:
- Hold an active PMHNP license in your home state with prescriptive authority.
- Get NLC privileges if your home state participates. This costs essentially nothing if your home state is already NLC.
- Add APRN-Compact privileges if applicable.
- Add direct state licenses one-by-one for high-population states that are not covered by the compact pathway you can use. Some employers may reimburse additional state licenses if they are sponsoring your hire; confirm the policy in writing.
Realistic cost: $200–$500 per non-compact state license, plus 8–14 weeks of processing time per state. Many telepsychiatry employers run their own credentialing teams and will handle the paperwork on your behalf.
DEA registration is its own animal
Every additional state where you wish to prescribe controlled substances may require separate state registrations, controlled-substance authority, or DEA-related steps in addition to your federal DEA. For a PMHNP this can matter significantly, especially when a role includes controlled-substance prescribing.
Two practical tips:
- Stagger your state DEAs by population. Start with the largest populations your employer needs (typically TX, FL, CA, NY).
- Plan for state and federal registration costs. Fees and reimbursement policies can change, so verify current amounts and employer reimbursement before accepting an offer.
Salary impact of being multi-state
Some market snapshots and job postings may show W-2 PMHNP telehealth salary ranges in the mid-six figures, but compensation varies by employer, state mix, credentials, schedule, productivity model, prescribing expectations, and employment type. Treat any $155k–$185k range or multi-state premium as directional and employer-specific, not guaranteed.
If you are negotiating, frame it explicitly: "I am licensed in N states covering ~X% of the US population, eliminating credentialing delays for your intake team." That is a number a hiring manager can take to a CFO.
What about SCOPE-OF-PRACTICE differences?
The APRN Compact does not change scope-of-practice rules in the receiving state. If you are PMHNP-licensed in a Full Practice Authority state but seeing a patient in a Reduced Practice state, confirm the receiving state's collaboration and prescribing requirements before providing care. Some multi-state telepsychiatry companies maintain in-state collaborating physicians to bridge this; verify the arrangement in writing and do not proceed until unclear requirements are resolved.
